Fahrsicherheitsrelevante Aufmerksamkeitsfunktionen nach Schlaganfall in der neuropsychologischen Rehabilitation

Zusammenfassung
Mobilität ist ein wichtiges Thema in der neuropsychologischen Behandlung. Bereits am Beginn der Rehabilitation ist die Fahreignung ein zentrales Anliegen von Schlaganfallpatienten. Aufmerksamkeitsstörungen beeinflussen die Fahrsicherheit in einem starken Ausmaß. Nach einem Schlaganfall treten häufig Aufmerksamkeitsdefizite auf. In einem ersten Schritt einer Studie wird mit 65 Schlaganfallpatienten ein Interview geführt, um die Bedeutung des Autofahrens und die Selbsteinschätzung der Fahreignung zu erheben. Anschließend erfolgt die Durchführung fahrsicherheitsrelevanter psychometrischer Tests. 29 Patienten mit einem erstmaligen ischämischen Schlaganfall in der Arteria cerebri media werden untersucht und einer Gruppe von Gesunden gegenübergestellt. Fünf Aufmerksamkeitstests werden in einen Score umgewandelt, der sich nach dem Ausmaß der Standardabweichung richtet. Auf Basis des Scores zeigen sich signifikante Unterschiede zwischen den gesunden Verkehrsteilnehmern und den Patienten. Schlaganfallpatienten überschätzen ihre geistige Fitness deutlich. Das Feedback anhand des Scores ist notwendig, um die Ergebnisse psychometrischer Tests transparent zu machen und dadurch die Einsicht zu fördern.

Attention deficits following stroke and ability to drive in neuropsychological rehabilitation

Abstract
Mobility is an important issue in neuropsychological treatments. Even in the initial phase of rehabilitation one of the most common concerns of stroke patients is the ability to drive. Safe driving is extremely sensitive to disturbances of attention. Attention deficits are very frequent following stroke. In the first step of a study 65 stroke patients are interviewed to analyze the importance of driving and the self-estimation on driving fitness. Afterwards psychometric tests related to driving performance are used. 29 patients with a first ischaemic stroke in the arteria cerebri media are assessed and compared with 14 healthy people. A set of five tests measuring attention functions is transfered in a score that is orientated on the standard deviation. The score-based results show significant differences between the healthy drivers and the patients. Stroke patients greatly overestimate
their cognitive fitness. The feedback in form of the score is necessary to make the results of psychometric tests transparent and to improve the awareness.
