Canadian Occupational Performance Measure (COPM) – patientenzentrierte Zielfindung und Outcome-Messung in der Ergotherapie

Zusammenfassung
Alltagsrelevanter Erfolg rehabilitationsneurologischer Maßnahmen wird meist am Grad der Unabhängigkeit des Patienten im Bereich der Selbstversorgung bemessen. Um eine umfassende Reintegration von Patienten in ihr früheres Lebensumfeld zu ermöglichen, sind jedoch auch Fertigkeiten in den Betätigungs-Bereichen »Produktivität« und »Freizeit« bedeutsam. Das COPM (Canadian Occupational Performance Measure [16, 19]) ist ein diagnoseunabhängiges ergotherapeutisches Assessment-Instrument, das für alle drei Bereiche der Betätigung erfaßt, welche Aufgaben der Patient aus seiner Sicht wieder durchführen muß oder möchte. Zugleich ermöglicht es die Messung des alltagsrelevanten Therapieerfolges aus Sicht des Patienten, und zwar so-wohl für seine Performance bzgl. der für ihn wichtigen Alltagsaktivitäten als auch für seine Zufriedenheit mit dieser Performance. Wir berichten über erste Erfahrungen mit diesem in Deutschland relativ neuen Assessment-Instrument. Anhand der Auswertung von 75 COPM-Interviews werden die Ziele der Patienten unserer Klinik dargestellt. Das COPM erfaßte hier deutliche Unterschiede von stationären im Vergleich zu ambulanten Patienten. Es maß signifikante Veränderungen von Performance und Zufriedenheit im Behandlungsverlauf. Im weiteren werden Faktoren genannt, die die Durchführung des COPM in der neurologischen Rehabilitation erschweren. Es wird gezeigt, daß das COPM nicht nur ein Meßinstrument darstellt, sondern daß der patientenzentrierte Ansatz auch die Motivation und Compliance des Patienten in der Ergotherapie fördern kann.

Canadian Occupational Performance Measure (COPM)

Abstract
The degree to which a neurological patient reaches physical independence in self-care activities in the course of rehabilitation is considered an important indicator of success of rehabilitation for his/her everyday life. However, to enable a complete reintegration of patients into their former psychosocial and environmental context, it is also important to include areas of occupation such as »productivity« and »leisure«. The COPM (Canadian Occupational Performance Measure [16, 19]) is a generic assessment for occupational therapists that identifies the problems and goals of patients in each of the three areas of occupation. In addition, it measures the subjective outcome of rehabilitation concerning occupations that are important to the individual patient, in terms of performance as well as satisfaction with the performance. The authors report on initial experiences in a neurological hospital with this assessment, which is quite new in Germany. Basing on a sample of 75 COPM-interviews, the goals of our patients are described. Regarding these goals, the COPM-interviews showed clear differences when comparing inpatients and outpatients. It measured significant changes of performance as well as satisfaction in the course of therapy. Furthermore, it has shown which factors may make the completion of the COPM-interview difficult in neurological rehabilitation. It turns out that the COPM is not only a suitable measuring instrument of occupational therapy, but that the patient-centred approach is also helpful in increasing motivation and compliance of patients in the occupational therapy process.
