Das Modellprojekt „Verwaltung aktiv – Teilhabe intensiv“ (VaTi)

Zusammenfassung

Der vorliegende Beitrag gibt einen Einblick in das Modellprojekt „Verwaltung aktiv – Teilhabe intensiv“ (VaTi). In VaTi wird ein innovativer Versorgungsansatz zur umfassenden Bedarfsermittlung und Teilhabekoordination für Menschen mit neurologischen Erkrankungen erprobt. Das Projekt zielt darauf ab, den Reha-Prozess zu verbessern, indem eine Versorgungslücke mit dem Ziel geschlossen wird, eine bestmögliche berufliche und soziale Teilhabe zu ermöglichen. Im Beitrag wird, neben der Darstellung von projektrelevanten Aspekten, die konkrete Umsetzung von VaTi anhand eines Fallberichts skizziert. Erste Ergebnisse der projektbegleitenden Evaluation unterstreichen den Bedarf einer Fallmanagement-basierten Teilhabekoordination für Betroffene und Angehörige nach dem Eintritt einer neurologischen Erkrankung. Möglichkeiten zur Verstetigung eines aufsuchenden Ansatzes und aktiver Teilhabekoordination im Reha-Prozess werden thematisiert.

The model project “Active Administration – Intensive Participation” (VaTi)

Abstract
This article presents an innovative care approach for comprehensive needs assessment and participation coordination for people with neurological disorders. This approach is examined as part of the project “Active Public Administration – Intense Participation” (in German: „Verwaltung aktiv – Teilhabe intensiv“, for short: VaTi). The project aims to improve the rehabilitation process by closing a care gap in order to enable the best possible vocational and social participation. In addition to presenting project-relevant aspects, the article outlines the concrete implementation of VaTi by using a case report. Initial results of the accompanying project evaluation emphasize the need for participation coordination based on case management after the onset of a neurological disease for those affected and their relatives. Opportunities for the institutionalization of an outreach approach and active participation coordination within the rehabili-tation process are being discussed.
