Der orthostatische Tremor – eine gelegentliche Ursache der Stand- und Gangunsicherheit in der neurologischen Rehabilitation

Zusammenfassung
Es wird über zwei Patienten berichtet, die eine seit vielen Jahren bestehende Unsicherheit beim Stehen beklagten, verbunden mit dem Gefühl zu stürzen. Das Gehen war hingegen zumindest anfänglich nicht beeinträchtigt. Bei auswärtigen Untersuchungen waren bislang eine Myopathie bzw. ein Normaldruckhydrozephalus als ursächlich angesehen und frustran behandelt worden. Im Rahmen einer stationären neurologischen Rehabilitation konnte aufgrund des typischen EMG-Befundes mit rhythmischer hochfrequenter Tremoraktivität der Beinmuskulatur im Stehen ein orthostatischer Tremor diagnostiziert und eine erfolgreiche Pharmakotherapie eingeleitet werden. Das Krankheitsbild könnte häufiger sein als bislang vermutet und sollte in der Differentialdiagnose der Stand- und Gangunsicherheit bedacht werden.

Orthostatic tremor – an incidental cause of postural instability and gait disorder in neurological rehabilitation?

Abstract
We report upon two patients with a long lasting history of progressive postural instability and the fear for falling. Walking, however, was initially not affected. A myopathy or normal pressure hydrocephalus were previously suspected. During a stay in a neurological rehabilitation ward we diagnosed primary orthostatic tremor. Surface electromyography in standing position showed characteristic tremor of high frequency in the leg muscles. The disease might be more frequent than usually supposed and should be taken into account in the differential diagnosis of gait and standing disorders.
