Praxisrelevante Meßvorrichtung zur Therapieobjektivierung der Elektrostimulation nach Eingriffen am Kniegelenk

Zusammenfassung
Die Elektromyostimulation ist ein wesentlicher Bestandteil der muskulären Stabilisierung nach Eingriffen am Kniegelenk. Durch eine therapiebegleitende Kraftmessung kann die Effektivität einer postoperativen Elektromyostimulation objektiviert werden. Es wird eine praxisrelevante Methode der Kraftmessung nach Eingriffen am Kniegelenk vorgestellt. Die entwickelte Meßvorrichtung eignet sich zur Bestimmung der isometrischen Maximalkraft (IMK) der Extensoren des Kniegelenkes. Es erfolgt ein Vergleich zwischen erkrankter und gesunder Extremität mit Angabe der gemessenen Seitendifferenz in Prozent. Bei 26 Patienten (12 männlich, 14 weiblich) wurde prä- und postoperativ (10.–20. Tag, 3.–4. Woche, 6.–8. Woche) die IMK der Knieextensoren im Seitenvergleich gemessen. Bei allen Patienten erfolgte die postoperative muskuläre Stabilisierung mit einem standardisierten Rehabilitationsprogramm unter Anwendung der Elektromechanotherapie nach Dippold. Bereits präoperativ bestand eine Seitendifferenz der IMK von durchschnittlich 20%. Die erste postoperative Messung (10.–20. Tag) ergab eine signifikante Zunahme auf 30%. Im weiteren Verlauf der postoperativen muskulären Stabilisierung mit standardisierter Elektromyostimulation (microstim®-Gerät) konnte die Seitendifferenz signifikant verringert werden. Bei 18 von 26 Patienten wurde 6 bis 8 Wochen nach Kniegelenksoperation eine nahezu seitengleiche Kraft der Knieextensoren gemessen. Mit der vorgestellten Meßvorrichtung ist eine therapiebegleitende Quantifizierung der muskulären Stabilisierung im Verlauf der Rehabilitation durch reproduzierbare Meßwerte möglich. Das Therapieziel besteht in der Beseitigung der meßbaren Seitendifferenz der IMK der Knieextensoren. Kurze Meßzeiten und eine leichte Bedienbarkeit ermöglichen eine routinemäßige Anwendung.

Measuring device for objectivizing electro-myostimulation after operations on the knee joint – relevant to the medical practice

Abstract
The electro-myostimulation is an essential part of muscular stabilization after operations on the knee joint. The efficiency of postoperative electro-myostimulation can be objectivized by measuring strength during therapy. A method of measuring strength after operations on the knee joint, which is relevant to the medical practice, is presented. The measuring device we have developed is suitable for determining the isometric maximal strength (IMK) of the extensors of the knee joint. A comparison is made between diseased and intact extremities showing the difference between the sides in percentages. With 26 patients (12 male, 14 female) the IMK of the knee extensors was measured pre- and postoperatively (10th–20th day, 3rd–4th week, 6th–8th week), with the two sides in comparison. A postoperative muscular stabilization based on a standardized rehabilitation programme applying electro-mechanical therapy according to Dippold was carried out with all patients. Even preoperatively a side difference of 20 per cent on average was found. The first postoperative measurement (10th–20th day) resulted in a significant increase up to 30 per cent. In the course of the postoperative muscular stabilization with standardized electro-myostimulation (using a microstim® device) the side difference in the IMK could be decreased significantly. An almost equal strength of the knee extensors was found with 18 out of 26 patients 6 to 8 weeks following an operation of the knee joint. With the help of the measuring device presented here it is possible to quantify muscular stabilization by reproducible measurements. This therapeutic method aims at eliminating any measurable difference between the sides in the IMK of the knee extensors. Routine application is facilitated by brief measuring times and easy handling.
