Ambulante Wohnform statt Pflegeheim – Entwicklungsmöglichkeiten bei Personen mit erworbenen Hirnschädigungen in der Langzeitrehabilitation

Zusammenfassung
Hintergrund: Die Zahl der überlebenden Patienten nach erworbenen Hirnschädigungen nimmt weiter zu. Betroffene müssen zum Teil mit erheblichen Einschränkungen leben und können ihr Reha-Potential aufgrund verkürzter Reha-Aufenthalte häufig nicht gänzlich ausschöpfen. Vielen, vor allem auch jüngeren Menschen, bleibt der Weg in ein Pflegeheim daher nicht erspart. Das Konzept des P.A.N. Zentrums der Langzeitrehabilitation versucht, die Patienten so weit zu fördern, dass sie im Anschluss ein möglichst selbstbestimmtes und eigenständiges Leben in einer ambulanten Wohnform führen können. Wie vielen dieser Schritt gelingt, ist Gegenstand dieser Untersuchung.
Methoden: Es handelt sich um ein Rehabilitationskonzept im 24-Stunden-Setting. Neben den wöchentlich 12,5 Stunden unmittelbar medizinischer Therapien findet in der übrigen Zeit pädagogische Teilhabeförderung statt. Es werden die Herkunft sowie die Wohnform nach Auszug der Patienten betrachtet. Ferner werden zu Beginn und beim Auszug der Frühreha-Barthel-Index (FRB) sowie das Mayo-Portland Adaptability Inventory (MPAI) zu zwei Zeitpunkten erhoben.
Ergebnisse: Es gingen 54 Patienten im Alter von durchschnittlich 44,76 Jahren in die Analyse ein. Die Aufenthaltsdauer betrug 15,61 ± 8,08 Monate (M ± SD). Insgesamt konnten über 90 % der Rehabilitanden den Weg in ein Pflegeheim umgehen. 66,67 % der Patienten wurden in eine Wohnform mit moderater Unterstützung und 9,26 % in eine Wohnform mit erhöhter Unterstützung entlassen.
Bei 24,07 % der Patienten stand weiterhin die Pflege im Vordergrund, lediglich 9,26 % mussten in ein Pflegeheim ziehen. Die FRB-Werte waren zum Zeitpunkt des Auszugs signifikant höher als zum Eintritt: Einzug = 31,30/Auszug = 50,28 (M). Beim MPAI verbesserten sich die Patienten in den Subskalen Fähigkeiten (p = 0,018), Partizipation (p = 0,018) sowie im Gesamtscore (p= 0,017) signifikant. In der Subskala Einstellung/Anpassung gab es keine signifikante Veränderung.
Schlussfolgerung: Durch eine gezielte Langzeitrehabilitation ist es möglich, auch den schwer betroffenen
Patienten so weit zu fördern, dass ein Pflegeheim verhindert werden kann – eine Grundvoraussetzung für ein partizipatives und möglichst selbstbestimmtes Leben.

Outpatient housing instead of nursing home – long-term rehabilitation options for persons with acquired brain injury

Abstract
Background: Due to advances in acute care medicine, survival rates from acquired brain injury are increasing. Some of the patients have to live with considerable limitations. Standard inpatient rehabilitation is often not long and/or comprehensive enough to ensure that the most severely affected patients can fully exploit their rehabilitation potential. In particular, for younger persons this may lead to a premature transfer to a care home setting. The P.A.N. Zentrum für Post-Akute Neurorehabilitation applies a holistic concept to prepare patients for, and help them through, the transition to an autonomous and independent life in an assisted-living setting. How many patients succeed in taking this step is the subject of this study.
Methods: The rehabilitation concept encompasses the whole 24-hour cycle. 12.5 hours are dedicated to medical and neuropsychological interventions, while the remaining time is devoted to activities that promote social participation and engagement. Outcome parameters comprise the relative change between the pre-incident and post-rehabilitation living arrangement. Quantitative data was obtained using the Frühreha Barthel Index (FRB) and the Mayo-Portland Adaptability Inventory (MPAI), recorded at the beginning and at the end of the inpatient rehabilitation period.
Results: Fifty-four patients with a mean age of 44 years participated in the study. The length of stay was 15.61 ± 8.08 months (M ± SD). In total, over 90 % of rehabilitants were able to avoid going to to a nursing home. 66.67 % were discharged into a setting with moderate support and 9.26 % into a form of living with increased support. Care measures were furthermore the main focus for 24.07 % of the patients, of which only 9.26 % had to move into a nursing home. FRB values were significantly higher after the rehabilitation: move-in = 31.30/ move-out = 50.28 (M). Based on the MPAI, there were significant improvements: ability (p = .018) and
participation (p = .018) subscales, as well as the total score (p = .017). No significant change was observed for the subscale adjustment.
Conclusion: Even in patients with an acquired brain injury with severe limitations, targeted long-term rehabilitation can help them transition to an independent living setting within the community. This is a basic
requirement for a participatory and autonomous life.
