Psychologische Aspekte der Fatigue

Zusammenfassung
Fatigue zählt im Rahmen der Multiplen Sklerose (MS) zu den häufigsten und belastendsten Symptomen. Sie kann sich bereits zu einem sehr frühen Zeitpunkt der Erkrankung manifestieren und stellt oftmals einen der Hauptgründe für soziale und berufliche Probleme dar. Die Pathophysiologie der Fatigue ist bis dato nicht geklärt. In Betracht kommen v.a. periphere, zentrale, neuroendokrinologische sowie immunologische Veränderungen, die im Sinne eines multikausalen Gefüges der Fatigue zugrunde zu liegen scheinen. Ein Modell der Ursacheninteraktion konnte allerdings bislang nicht aufgestellt werden. Die einzige Möglichkeit einer Symptomerfassung stützt sich derzeit auf Selbstbeurteilungsverfahren. Mit ihrer Hilfe sollen die wesentlichen Aspekte der Fatigue und ihr Einfluss auf Alltag und Lebensqualität quantifiziert werden. Ein zuverlässiges Instrument zur Erfassung befindet sich momentan in der Validierungsphase und soll zukünftig helfen, Fatigue in der klinischen Routine zu charakterisieren.

Abstract
In multiple sclerosis (MS) fatigue belongs to one of the most common and most disabling symptoms. It can be present already at the very early stages of the disease and often represents one of the principal reasons for social and occupational problems. So far, the pathophysiology of fatigue is unknown. Peripheral, central, neuroendocrinological as well as immunological alterations were considered to underlie fatigue in terms of a multicausal system. However, a model explaining the causal interactions between these factors has not yet been proposed. At present, the assessment of the symptom is solely based on self-report measures. By means of fatigue questionnaires the essential aspects of fatigue and their influence on everyday life and quality of life are to be quantified. One recently developed instrument to assess fatigue reliably is currently under validation and might help in the future to diagnose and clearly characterise the symptom during clinical routine.
